Integrated Team Care

The Integrated Team Care Program (ITC) is designed to support individuals who identify as Aboriginal or Torres Strait Islander's that have a chronic illness, to manage and understand their condition. Through ITC you have access to a  an Aboriginal Health Worker, that can provide guidance and support in a team arrangement with your chosen general practitioner. The program works toward a holistic person centered approach to health and wellbeing. The service provides transport, support and assistance in navigation of the health system. It can provide referral, advocacy, education in line with chronic health conditions and assist in following health plans set by health professionals.

A men’s health and wellbeing group. group activities are held around health, mental health, nutrition and culture. The program prides itself in being an inclusive group that is very supportive of each other, within a comfortable setting.
