Youth Mental Health & Family Violence Coordinator - Full Time
Youth Mental Health & Family Violence Coordinator - Part time
​
Positions Available for Full Time or Part Time for Youth Mental Health & Family Violence Coordinator. This role is to raise awareness of mental health to the youth (12 - 25 years) in the region and the impact it has on individual family and community level and reduce barriers to youth accessing services.

Strengthen the emotional wellbeing of Aboriginal and Torres Strait Islander youth by providing healing activities and supports;
-
Delivering individual case management services and providing referral pathways for families;
-
Deliver workshops and educational sessions that provide resources to address mental health;
-
Develop and coordinate programs and report accordingly, including budgeting expenses;
-
Maintain North West Tasmania Aboriginal culture through “On Country” experiences;
-
Ensure all client information is kept secure and safe and confidentiality is of high priority and non-negotiable;

Work effectively in team environments

Aged Care & NDIS Manager
Aged Care and NDIS Manager - Full Time
​
The Full Time position of Aged Care & NDIS Manager leads and oversees the aged care and disability support services to approved clients who choose Circular Head Aboriginal Corporation as their aged care or disability support service provider.
-
Leading and managing CHAC’s Aged Care and NDIS Support service to ensure quality and professional services are provided that meet the needs of CHAC’s Board, Executive, clients, funding bodies and other service providers, in accordance with contractual requirements;

Managing the day-to-day operations of the Aged and NDIS Programs to ensure quality care is provided that meet the needs of clients;
-
Managing the financial costings, budget plans and negotiation of external contracts for CHAC’s Aged Care and NDIS Support service and monitor budget versus actual for these services/programs;
-
Recruiting to vacant positions within the program when and if they occur;

Home Care Coordinator
Home Care Coordinator - Full Time
​
The Full Time position of Home Care Coordinator is primarily the coordinated assistance to the aged care and MAIB support services to CHAC approved clients who choose our organisation as their aged care service provider.

Reporting directly to the Aged Care & NDIS Manager

Responsible for the day- to-day coordination of the Aged Care Service and MAIB client service to ensure quality care is provided that meet the needs of client
-
Rostering home carers and support workers in accordance with the Award to deliver scheduled care according to care plans
