INTEGRATED TEAM CARE (ITC)
Primary Health Tasmania has commissioned Circular Head Aboriginal Corporation (CHAC) to deliver the Integrated Team Care program from 2017.
The program is funded by the Australian Government under the Indigenous Australians’ Health Programme, and delivered according to the Government’s guidelines.
The focus of the program is to assist those who identify as Aboriginal or Torres Strait Islander living with a chronic health condition to better co-ordination of care.
ITC – have one full time Care Co-ordinator/ Registered nurse, who can provide intensive one-to-one support to access services and assist you navigate and understand the health system. Clinical care can also be provided in the comfort of home or in a culturally safe environment at CHAC health centre. This service is conducted in consultation with the management plan and the treating General Practitioner (GP).
ITC- have one full time Outreach worker whom is non-clinical staff with strong links to the community. They work closely with the care coordinator and provide support such as helping people travel to their health appointments.
This service is free and requires referral from a GP and construction of a GP management plan and or Team care arrangements. ITC can assist to gain this referral and are more than happy to attend medical appointment to support clients.
